Significant progress has been made of the molecular lesions responsible for tumor cells to exhibit uncontrolled growth while circumventing normal mechanisms of apoptosis and their ability to migrate and invade normal tissues while evading recognition and destruction by the immune system. This understanding has enabled the development of therapies specifically targeted to these lesions coupled to innovative treatment regimens to most effectively use these new targeted therapies with precision in selected subpopulations of patients. But at the same time the cost of personalized medicine in oncology is increasing and the varied and contrasting priorities pharmaceutical industry, local and national governments, health technology assessment bodies, medical community, and patients need to be balanced to ensure access to innovative oncology therapies. With the rapidly changing environment of health care delivery and the numerous precision medicine studies it is essential to study how data from both observational studies and clinical trials can be used to provide evidence on health outcomes in real-world practice settings.
